Terms and Conditions for Assessment
A deposit of £100 will be taken to secure the booking.
All questionnaires need to be completed and returned before the assessment day.
Proof of a recent sight test will be required, and parents will be asked to complete the visual difficulties screening protocol.
Full payment will be required on or before the day of assessment.
Cancellation more than 24 hours, deposit will be returned at the assessor's discretion.
Rescheduling will be considered if done in advance.
If the assessor cancels, I will reschedule at your earliest convenience.
The assessment is designed to determine whether an individual has dyslexia. It does not specifically assess for dyscalculia, dyspraxia, ADHD, or ASD. If co-occurring difficulties arise during the assessment, a referral to an appropriate specialist or a GP may be recommended.
The full report will be completed and distributed within a month of the assessment.
All personal data will be handled in compliance with the Data Protection Act and GDPR (2018).
Following the assessment and the compilation of the final assessment report all client questionnaires, all raw data test sheets and all other personal data will be permanently deleted/destroyed.
